Most cosmetic practices don’t have a lead problem — they have a conversion problem.

You’re spending thousands of dollars on ads to generate leads, but if those leads aren’t converting into paid surgeries, you’re burning time, energy, and money.

In today’s market, patients are savvier, competition is fiercer, and attention spans are shorter than ever. So the real question is: How do you convert more consultations into surgical bookings?

After two decades consulting with aesthetic practices, I’ve identified five key areas that drive conversions — and most of them have nothing to do with your surgical skills.

Let’s break them down:

1. Have the Right Person In the Right Role

Your patient coordinator isn’t just a scheduler. She’s your closer. Her job is to guide patients from “just looking” to confidently saying, “Yes, I’m ready.”

That means she must be:

  • Warm, approachable, and likable
  • Clear that her job is not to sell, but to serve
  • Able to build trust quickly by being informed, calm, and emotionally intelligent

This is someone asking a stranger to hand over thousands of dollars for something deeply personal. That takes serious skill and presence.

2. Set Clear Goals and Offer Incentives

If you want better performance, set clear metrics and reward results.

Vague goal: “Keep me busy in surgery.”
Better: “Convert X number of consults to booked cases each month.”

Use past data to set a baseline quota. For example, count up all surgical revenues for the past 24 months, divide that amount by 24 and now you have the average monthly revenue that your practice knows how reach – because you’ve done it. This is the minimal goal for your coordinator to reach since that is what her salary is based on.

Then offer your coordinator performance-based incentives for “above and beyond” the base salary and quota. So, whenever she converts more, she can be rewarded for either a flat bonus per surgery booked or a percentage of collected revenues.

And have the bonus paid out monthly, not quarterly. Top performers produces more when reward is immediate.

But what if the patient cancels surgery?  If a patient cancels or requests a refund, claw the bonus back from the coordinator. This encourages coordinators to attract quality patients who follow through.

3. Give Her The Right Tools

You can’t expect strong conversions if your coordinator is buried in Excel sheets and sticky notes.

Invest in a modern CRM that:

  • Tracks leads from inquiry to booking
  • Automates reminders and follow-ups
  • Houses notes, preferences, and email/text templates
  • Makes accountability easy

Let technology handle the repetitive tasks so your coordinator can focus on building relationships and closing consultations.

And, give her professional training. Nobody is a born “closer”. Like anything else, this takes strategic know-how, practice and accountability to become a top revenue producer for a practice.

To uplevel even more, consider implementing these strategies to step up your game to significantly change your conversion rate:

Create A Space That Converts

Give your coordinator a private office to do her job effectively.

Too often I see practices where the coordinator is roaming from  exam room to exam room, balancing a laptop on her knees, juggling folders and presenting quotes to patients from no real base to work from.

That’s not a power position — that’s a traveling salesperson.

Your consultation room matters more than you think. It should feel private so the patient feels comfortable sharing financial and other personal details out loud.

Enhance this office with subtle supportive signage such as “Why Choose Us?”, awards and wall art of happy patients who love their results.

Pro Tip: Your coordinator starts the consult behind the desk (authority position) and then moves beside the patient (partnership position) when presenting the quote. This small shift increases trust and conversions.

Turn Your Coordinator Into a Walking/Talking Testimonial

One of the fastest ways to boost conversions? Have your coordinator become a patient.

When she’s had a procedure herself, she becomes living proof of your results. She’s more confident, more relatable, and more persuasive.

I’ve seen coordinators triple their results overnight because she knows exactly what the emotional and physical journey is like and can relay that with empathy to prospective patients to help them overcome their fear.

Handle Price Objections Strategically

Patients aren’t just comparing you to another surgeon — they’re weighing their options between you and a vacation, furniture, tuition, etc.

They will oftentimes try for a discount. Rather than be offended or defensive, use these strategies to meet them in the middle, by offering “purposeful incentives” with a reason behind them.

Offer group-specific promos so these patients feel special and part of the community:

  • Teachers
  • Nurses
  • Gym members
  • Local realtors
  • Dog lovers
  • Yogis, etc.

And, or offer value-adds instead of discounts. For example:

  • “Dr. Smith can remove those moles at no charge during surgery.”
  • “He can revise your old scar that bothers you while you’re under.”

These small gestures feel huge and generous to the patient and cost you next to nothing.

And, patients love to spend money to save money — so lean into that psychology. If you don’t discount, at least help them find the money using creative options such as:

  • 5% discount for payment in full at the consultation
  • 5% “cash savings” instead of charging a credit card fee
  • Monthly financing options to break $20K into manageable chunks

And please — don’t let 3% credit card fees block you from collecting a $15K case.
That’s called, “Tripping over thousands to save pennies.”

Use Texting to Convert Faster

Today’s patients live on their phones — and texting is fast, convenient, and effective. It also creates urgency when used strategically.

For example, if you have a sudden opening in your surgery schedule you need to fill right away, text everybody who had a consult but is on the fence with this message:

“Hi Sarah, it’s Kim from Dr. Smith’s office. A surgery spot just opened up. Are you fee to chat?”

It creates urgency and feels personal. I’ve said yes to those kind of texts myself 😉

Final Thought

If your leads aren’t converting, don’t immediately spend more on ads.

Instead, look at your conversion process:

  • Is your coordinator trained, supported, and incentivized?
  • Does she have the tools to track and follow up effectively?
  • Are you warming up patients before the consult?
  • Is your space and pricing strategy working with you — or against you?

Small changes here make a huge difference.
